• What is my deductible, and has it been met?

• How many sessions per year will be reimbursed?

• What amount will be reimbursed for therapy sessions with an out-of-network provider?

• Is approval required by my primary care physician?

• What is the process for submitting the detailed monthly invoice (also called “a superbill”)?
